nur sinnvoll, weil sich die Effekte der Therapie aus den
Bedingungen und Prozessen jedes Einzelfalls heraus ergeben, sondern
hat sich auch substanziell geändert. Die Möglichkeiten der
Digitalisierung haben über Internetund App-basierte Technologien
die Erfassung von Outcome und Prozessen einfach gemacht und geben
bei entsprechend hochfrequenten Abtastraten auch Einblick in den
aktuellen Verlauf. Damit leistet Evaluation einen Beitrag zur
aktiven Gestaltung von Psychotherapien, über Prozessfeedback und
-reflexion mit Einbezug der Patient*innen auf Augenhöhe. Für diese
Möglichkeiten der Prozessund Outcome-Evaluation wurde das
Synergetische Navigationssystem (SNS) entwickelt, das
Datenerfassung, -speicherung und -analyse kombiniert. Patient*innen
können über eine App quantitative Einschätzungen (z. B.
Skalierungen) und elektronische Tagebucheinträge vornehmen, die auf
einem Bildschirm im Form von Zeitreihen und Textfeldern dargestellt
werden. Verschiedene Methoden geben Einblick in die nichtlinearen
Eigenschaften therapeutischer Prozesse (z. B. kritische
Instabilitäten) und in die stattfindenden Musterwechsel. Das SNS
erfüllt damit Funktionen der Therapierevaluation, der
Prozessreflexion und -steuerung sowie der Praxisforschung. Neben
den verfügbaren standardisierten können auch personalisierte
Fragebogen erstellt werden, deren Items sich aus einer von
Therapeut*in und Patient*in gemeinsam durchgeführten Fallkonzeption
(z. B. der idiografischen Systemmodellierung) ergeben.
Abstract:
Process and outcome in psychotherapy can be seen as results of a
self-organizing system which includes all factors contributing to
success or failure in a specific case: patient and therapist
factors, the therapeutic alliance, the setting, the living
environment of the patient, interventions and all intended or
unintended treatment components. In consequence, evaluation has to
be implemented in real-world practice, not only in specific
studies. New methods of digitalized outcome and process monitoring
like the Synergetic Navigation System (SNS) facilitate the
assessment and the analysis of self-organized dynamics and outcome
in psychotherapy. In routine-practice, daily self-assessments
mirror the complex dynamics of change and deliver the data base for
the automatically applied nonlinear analysis methods on time
series. The visualization of the therapeutic process on a computer
screen allows the cooperative reflection of the dynamics by patient
and therapist (continuous cooperative process management). By this,
the evaluation of processes actively contributes to change dynamics
and is a learning process of its own. By using the SNS not only
standardized outcome and process questionnaires can be applied but
also personalized questionnaires which are created by the patient
and the therapist from a systemic case conceptualization
(idiographic system modelling).
Abstract:
La valutazione della pratica psicoterapeutica standard non è solo
utile in quanto gli effetti della terapia sono influenzati dalle
condizioni e dai processi adottati nei singoli casi, ma anche
perché le modalità di valutazione sono cambiate nella sostanza. Le
nuove possibilità offerte dalla digitalizzazione sviluppata grazie
alle recenti tecnologie relative a internet e alle app hanno
facilitato la raccolta dei risultati e dei dati riguardanti i
processi delle pratiche terapeutiche e forniscono, sulla base dei
campionamenti frequentemente rilevati, un quadro sugli sviluppi
attuali. In tal modo la valutazione apporta un contributo
importante per una formazione attiva delle psicoterapie, per
ricevere feedback e formulare riflessioni relativamente ai processi
impiegati coinvolgendo alla pari il paziente. Il sistema di
navigazione sinergico (SNS), che combina raccolta, salvataggio e
analisi dei dati, è stato sviluppato proprio per queste possibilità
di valutazione dei risultati e dei processi adottati nelle
psicoterapie. I pazienti possono dare delle valutazioni
quantitative tramite app (ad esempio indicare un valore in una
scala) e scrivere annotazioni in un diario elettronico che poi
potranno essere visualizzate su schermo sotto forma di serie
temporali o campi di testo. I diversi metodi forniscono un quadro
delle caratteristiche non lineari dei processi terapeutici (ad es.
instabilità critiche) e dei cambiamenti di paradigma avvenuti.
L’SNS soddisfa in tal modo le varie funzioni che comprendonodi
valutazione terapeutica, formulazione di riflessioni, gestione dei
processi nonché ricerca nell’ambito della pratica terapeutica.
Oltre ai questionari standardizzati disponibili, è possibile
redarre anche dei questionari personalizzati le cui voci derivano
da un’analisi elaborata dal terapeuta insieme al paziente (ad es.
creazione di modelli di sistema idiografici).
